Video of PES Refinery Leak and Explosion; Sizzler Rides Shutdown in NJ; Fake Nurse Arrested

Officials have released video from inside the PES refinery in South Philadelphia just before it exploded on June 21. A ground-hugging cloud of hydrofluoric acid can be seen creeping along before catching fire. The U.S. Chemical Safety Board found that an elbow in a pipeline hadn't been replaced in decades and the thickness of that pipe had degraded well below replacement levels. That is the likely cause of the gas leak.



New Jersey state officials have ordered the shutdown of all rides similar to the one involved in a deadly accident at a festival in Cumberland County last weekend. 10-year-old Hailey McMullen was killed after being thrown from a Wisdom Super Sizzler ride named "Xtreme." It happened on Saturday evening, 10/12, at the Deerfield Township Harvest Festival. The Department of Community Affairs is still investigating the cause of the accident. The ride's state inspection records were in compliance and up-to-date. A public viewing for Hailey will be held on Friday, 10/19, from 6 p.m. to 9 p.m. at Maranatha Baptist Church in Millville.

Philadelphia Police announced that a West Chester woman has been arrested for fraudulently practicing as a registered nurse. 48-year-old Rachel Ortega, aka Rachel Bordman, was arrested on Thursday, 10/18. She was charged with theft by deception and impersonating a notary public. Authorities say that Ortega has never been authorized to practice as a licensed nurse in the state of Pennsylvania. However, she was operating a business named Shanahan Enterprises, LLC and providing services as a home health care nurse to members of the community.
